How to register with the practice
To register with the surgery you will need to complete a (GMS1), a surgery registration form and a permission to share or leave messages form which you can collect from reception or download using the links below.
You will not be formally registered until you have brought these into the surgery and we have seen the following identification.:
- Photo identification such as your passport of driving license
- proof of your address, such as a recent utility bill (Gas, Electric, Water or landline phone bill or council tax
When you register you will also be offered a 'new patient' medical check. This is because it can take a considerable time for us to receive your medical records and will help us understand your current medical needs.

If you would like messages to be left on your answer phone in the event that the Practice may need to contact you, would like someone else to ring on your behalf, or nominate someone to pick up prescriptions on your behalf, please complete the 'Permission to share or leave messages' form.
